超声在新生儿颅脑损伤中的诊断价值

Diagnostic value of ultrasound in the neonatal injured brain.

摘要: 目的 研究超声对新生儿颅脑损伤的诊断价值。方法 选取2013年5月-2015年8月本院新生儿科住院,有围生期窒息病史的患儿80例,产前超声已排除颅内先天结构异常,于出生72 h内行颅脑二维超声检查,并经颞囟检测大脑前动脉和大脑中动脉血流动力学变化,主要参数为收缩期峰值血流速度(Vs)、舒张末期血流速度(Vd)以及阻力指数(RI)。于超声检查结束24 h内行头颅磁共振检查,以磁共振结果为金标准,比较分析超声检查结果。结果 80例患儿磁共振检查均提示脑损伤,呈多灶性分布。超声检查62例表现不同程度的脑损伤,阳性率为77.5%。结论 颅脑超声可早期发现新生儿脑组织结构改变及血流动力学变化,为临床诊断提供重要信息。

Abstract: Objective To explore the diagnostic value of ultrasound in neonatal brain injury. Methods Totally 80 cases with perinatal asphyxia history,were selected from May 2013 to August 2015 in our pediatric hospital,prenatal ultrasound had excluded intracranial congenital structural abnormalities,cranial ultrasound was conducted at 72 h of birth,to detecte cerebral artery and middle cerebral artery blood flow dynamics from the temporal anterior fontanelle,the main parameters were peak systolic velocity (Vs),diastolic velocity (Vd) and resistance index (RI).At the end of 24 hours,head MRI was used as the gold standard to compare with the ultrasound results. Results 80 cases of children were prompted MRI brain injury,showed multifocal distribution.62 cases of ultrasound showed varying degrees of brain damage,the positive rate was 77.5%. Conclusions Early ultrasound of neonatal cranial can detect structural and hemodynamic changes in cerebral tissue,provide important information for clinical diagnosis.
