纯母乳喂养与低龄儿童龋发生关系的Meta分析

Meta-analysis of the relationship between exclusive breastfeeding and dental caries in young children

摘要: 目的 研究纯母乳喂养与低龄儿童龋发生之间的关系,为未来低龄儿童龋病的早期预防提供依据。方法 通过中文和外文的数据库检索1979—2017年8月的文献。中文库包括中国期刊全文数据库、万方数据库和维普数据库,外文库包括PubMed、Science Direct OnSit、Elsevier、ProQuest、Cochrane和Wiley-BlackWell。纳入比较纯母乳喂养、混合喂养和人工喂养儿童低龄儿童龋发生结局的横断面研究12篇,使用Rev Man 5.3进行异质性检验,采用固定或随机效应模型进行Meta分析。结果 Meta分析发现与人工喂养的儿童相比,纯母乳喂养与低龄儿童龋发生的总相关RR值达0.62,95%可信区间为0.48~0.80;与混合喂养的儿童相比,纯母乳喂养与低龄儿童龋发生的总相关RR值达0.73,95%可信区间为0.60~0.88。未发现纯母乳喂养与重度低龄儿童龋发生之间的关联。结论 纯母乳喂养对低龄儿童龋的发生具有保护作用,未来在低龄儿童龋早期预防方面需要重点关注母乳喂养模式,推广纯母乳喂养。

Abstract: Objective To study the relationship between exclusive breastfeeding and dental caries in young children, in order to provide evidence for early prevention of dental caries in children . Method Papers were searched through databases of both Chinese (including Chinese Journal Full-text Database, Wanfang Database and Weipu Database) and English (including PubMed, Science Direct OnSit, Elsevier, ProQuest, Cochrane and Wiley-Black Well) from 1979 to August 2017. Totally 12 cross-sectional studies on the incidence of dental caries in young children with exclusive breastfeeding, mixed feeding and artificial feeding were included in this study. The heterogeneity test was performed by Rev Man 5.3 and Meta-analysis was performed using a fixed or random effect model. Results Meta-analysis showed that the total correlation RR value of exclusive breastfeeding and dental caries in young children was 0.62 (95%CI: 0.48~0.80) compared with artificial feeding children, and was 0.73 (95% CI: 0.60~0.88) compared with mixed feeding children. The relationship between exclusive breastfeeding and severe dental caries in young children was not found. Conclusions Exclusive breastfeeding has a positive effect on the prevention of early childhood caries, and it is necessary to promote exclusive breastfeeding.
