哮喘儿童外周血MCP-4、MDC、CysLTs检测及临床意义

Levels and clinical significances of MCP-4,MDC and CysLTs in peripheral blood of children with asthma.

摘要: 目的 探讨哮喘患儿外周血单核细胞趋化蛋白4 (monocyte chemoattractant protein 4,MCP-4)、巨噬细胞衍生趋化因子(macrophage derived chemokine,MDC)、半胱氨酰白三烯(cysteinyl leukotrienes,CysLTs)和嗜酸性粒细胞(eosinophils,EOS)水平,以了解它们在儿童哮喘发病中的作用及临床意义。 方法 采用酶联免疫吸附法(ELISA) 和血细胞分析仪分别测定35例哮喘急性发作期儿童 (哮喘发作组)、32例哮喘缓解期(哮喘缓解组)、32例肺炎(肺炎组)及25例健康儿童(健康对照组)血清中MCP-4、MDC、CysLTs水平和外周血EOS数,并进行分析。 结果 哮喘发作期组患儿血清MCP-4、MDC、CysLTs水平和外周血EOS明显高于哮喘缓解组、肺炎组及健康对照组(P均< 0.05);哮喘缓解组EOS数仍高于肺炎组及健康对照组(P均< 0.05),MCP-4、MDC、CysLTs水平均高于健康对照组(P均< 0.05),三者水平与肺炎组比较差异无统计学意义(P均>0.05);MCP-4、MDC、CysLTs 水平及EOS相互之间呈正相关。 结论 MCP-4、MDC和CysLTs参与了儿童哮喘发病,并与病情严重程度有关,其水平可作为儿童哮喘诊断和病情评价指标,并对指导治疗具有重要的临床价值;哮喘缓解期仍存在慢性炎症反应。

Abstract: Objective To investigate the levels of monocyte chemoattractant protein 4 (MCP-4),macrophage derived chemokine (MDC),cysteinyl leukotrienes (CysLTs) and eosinophils (EOS) in peripheral blood of children with asthma,in order to understand their roles in the pathogenesis of asthma and clinical significances. Methods Serum levels of MCP-4,MDC,CysLTs and EOS in 35 children with asthma exacerbation (asthma exacerbation group),32 children with asthma convalescence (asthma convalescence group),32 children with pneumonia (pneumonia group) and 25 health children (control group) were measured by enzyme linked immunosorbent assay (ELISA) and blood cells analyzer respectively,data were analysed. Results Serum levels of MCP-4,MDC,CysLTs and EOS in asthma exacerbation group were significantly higher than those in asthma convalescence group,pneumonia group and control group (P<0.05).The level of EOS was higher in asthma convalescence group than that in pneumonia group and control group (P<0.05).The levels of MCP-4,MDC,CysLTs in asthma convalescence group were higher than those in control group (P<0.05),but there were no significant differences compared with pneumonia group (P>0.05).The levels of MCP-4,MDC,CysLTs and EOS were associated with the severity of the disease,and there were positive correlations between them. Conclusions MCP-4,MDC and CysLTs are all involved in pathogenesis of asthma in children,and related to the severity of asthma,so their serum levels and counts can be used for diagnosis and evaluation of severity of asthma in children,and it is of great value to clinical treatment;Chronic inflammation still exists in convalescence phases of asthma.
