乌司他丁对心肺脑复苏后血细胞因子与脑超微结构的影响

Effects of ulinastatin for injection (UTI) on cytokines and ultrastructure of brain tissue after cardiopulmonary-cerebral-resuscitation

摘要: 目的: 探讨酶抑制剂乌司他丁(ulinastatin for injection, UTI) 对心肺脑复苏后肿瘤坏死因子-α(TNF-α) 、白细胞介素-6(IL-6) 、白细胞介素-10(IL-10) 和脑超微结构的影响。方法: 将24 只成年健康普通级SD 大鼠, 随机分成对照组、复苏组和乌司他丁组, 每组8 只。对照组仅行气管切开术, 不夹闭气管, 不造成心跳呼吸骤停;复苏组与乌司他丁组建立心肺脑复苏模型, 乌司他丁组在心肺脑复苏时加用单次UTI 106 U·Kg-1。对照组在气管切开即刻、0. 5 、2 、4 、6 h 采集血标本, 复苏组与乌司他丁组分别在气管切开即刻、复苏成功后0. 5 、2 、4 、6 h 采集血标本, 以放免法测定血清中TNF-α、IL-6 、IL-10 的水平。实验结束时每组随机取2 只动物快速断头取皮层脑组织观察脑超微结构的变化。结果: 对照组中TNF-α、IL-6 、IL-10 基本平稳, 各时间点间无显著变化(P >0. 05) 。复苏组复苏后TNF-α、IL-6 明显上升, IL-10明显下降(P<0. 01 或P<0. 05) 。乌司他丁组TNF-α、IL-6 、IL-10 复苏后各时间点与气管切开即刻比较均无显著性差异(P >0. 05), TNF-α在复苏后0. 5 、2 、4 h 与复苏组同时间点比明显下降(P<0. 05), IL-10 在复苏后0. 5 、6 h 与复苏组同时间点比明显升高(P<0. 05) 。脑组织超微结构改变:乌司他丁组较复苏组明显减轻。结论: 复苏过程合用UTI 可以抑制TNF-α、IL-6 的释放, 增加IL-10 的释放, 减轻脑组织损伤, UTI 可能通过干预这些炎性细胞因子对心肺脑复苏后组织器官起到一定的保护作用。

Abstract: AIM: To investigate the effects of ulinastatin for injection (UTI) on the levels of TNF-α, IL-6 and IL-10, and ultrastructure of brain tissue in rats after cardiopulmonary-cerebral-resuscitation (CPCR).METHODS: 24 SD rats were randomly divided into three groups (n =8 in each): control group, resuscitation group and UTI group. The levels of TNF-α, IL-6 and IL-10 were quantified by isotope radio immunoassay at different time before and after resuscitation, and the changes of ultrastructure of brain tissue were observed after the experiment.RESULTS: The plasma concentrations of TNF-α and IL-6 increased (P<0. 01, or P<0. 05) and the plasma concentration of IL-10 decreased (P<0. 05) after CPCR in the rats. There was no significant change (P > 0. 05) at different time points in the control group and in the plasma concentrations of TNF-α, IL-6 and IL-10 after CPCR in UTI group (P >0. 05). The plasma concentration of TNF-αshowed a significant change in 30 minutes, 2 hours and 4 hours compared with the resuscitation group, and the plasma concentration of IL-10 in 30 minutes and 6 hours compared with the resuscitation group. The observation of the ultrastructure in brain tissue showed that the injury was relieved obviously in the UTI group than the resuscitation group.CONCLUSION: UTI may prevent the damage of organs caused by CPCR by inhibiting TNF-αand IL-6, and enhancing IL-10.
