盐酸戊乙奎醚对单肺通气时肺内分流及炎症反应的影响

Effect of penehyclidine hydrochloride on intrapulmonary shunt and inflammatory factors during one lung ventilation in patients receiving thoracic surgery

摘要: 目的: 观察盐酸戊乙奎醚(长托宁)对单肺通气(OLV)时肺内分流,炎性细胞因子和自由基的影响。方法: 拟行肺叶切除术肺癌病人30例,随机分为长托宁组(A组)和对照组阿托品组(B组),每组15例,两组于麻醉诱导前10 min,分别给予长托宁 1 mg 静注,阿托品 0.5 mg 肌注。两组病人分别在插管后双肺通气 5 min(T1)、OLV 45 min(T2)、90 min(T3)及术后 2 h(T4)、24 h(T5)采取动脉血,作血气分析,计算肺内分流率(Qs/Qt)。测定血清肿瘤坏死因子-α(TNF-α)、白细胞介素IL-6、IL-8浓度、超氧化物歧化酶(SOD)活性及丙二醛(MDA)浓度。结果: 两组患者单肺通气时SPO2、PaO2均较双肺通气时(T1)降低,Qs/Qt较T1增加(P<0.05);B组T2-4时SPO2、PaO2较A组下降明显,Qs/Qt较A组升高(P<0.05);两组TNF-α、IL-6和IL-8于T2时明显上升(P<0.05),与B组比较,A组T3、T4、T5时TNF-α、IL-6和IL-8明显低于B组(P<0.05);两组MDA在T3、T4、T5时显著升高,SOD同时呈下降趋势(P<0.05);组间比较,B组MDA在T3、T4时显著高于A组(P<0.05)。B组SOD在T3、T4、T5时较A组降低(P<0.05)。结论: 长托宁可改善单肺通气病人肺内分流及降低炎性反应。

Abstract: AIM: To observe the effect of penehyclidine hydrochloride on intrapulmonary shunt and inflammatory factors in lung cancer surgery.METHODS: Thirty patients with lung cancer undergoing lung resection were randomly divided into two groups with 15 cases each: penehyclidine hydrochloride group (group A) and atropine group (group B). Blood gas analysis, TNF-α, IL-6, IL-8, SOD and MDA concentrations were detected at two lung ventilation(TLV) (T1, baseline), OLV 45 min(T2), OLV 90 min (T3) and 2 h, 24 h after operation(T4, T5).RESULTS: SPO2, PaO2 were lower in group B than those in group A, Qs/Qt was higher than group A at T2-4 (P<0.05). Serum TNF-α, IL-6, IL-8 SOD concentrations were lower at T3-5 in group A than those in group B (P<0.05). Serum MDA concentration was higher at T3-4 in group B than in group A.CONCLUSION: Penehyclidine hydrochloride could reduce intrapulmonary shunt and alleviate the pulmonary inflammatory reactions in lung cancer surgery.
