羟乙基淀粉对失血性休克大鼠脑的保护作用

Protective effect of Hydroxyethyl Starch on hemorrhagic-shocked rat brain

摘要: 目的 研究羟乙基淀粉200 /0.5 注射液(Hydroxyethyl Starch, HES) 对失血性休克大鼠脑的保护作用。方法 25 只SD 大鼠随机分为对照组(A组) 、休克组(B 组) 、全部自体血复苏组(C 组) 、2 倍出血量的HES (超常量, >33 mg·kg-1 ) 复苏组(D组) 、1 倍出血量的HES(常规量, ≤33 mg·kg-1 ) 加1/2出血量的自体血复苏组(E 组) , 每组5 只。采用大鼠股动脉放血休克模型, 休克1 h 后分别输入上述液体, 复苏30 min 后取脑。脑组织切片行HE 染色观察脑水肿和炎性细胞浸润的程度;用免疫组化染色和Western-bloting 法检测脑组织中NF-κB 的核移位及其表达活性。结果 在休克1 h 时脑组织NF-κB 的表达即有增加;C 组和D 组NF-κB 的表达增加明显, 脑水肿和炎性细胞浸润较严重;E 组NF-κB 的表达增加下调, 脑水肿和炎性细胞浸润亦较轻。各组NF-κB 的核移位与脑组织中NF-κB 的表达活性变化基本一致。结论 在急性失血性休克(出血量为估计总血量的35 %) 时, 大鼠脑组织中NF-κB 的核移位和表达增加;常规量HES 复苏使NF-κB 表达增加下调、核移位减少、脑水肿和炎性细胞浸润亦减轻,对脑可能具有保护作用。

Abstract: AIM: To investigate the protective effect of 6 %Hydroxyethyl Starch (HES, 200 0.5) on the brain of hemorrhagic-shocked rats.METHODS: Sprague-Dawley rats were randomly divided into sham operation group (group A) , hemorrhagic shocked group(group B) , whole blood return group(group C) , HES at 2x shed blood volume(SBV) group(group D) , HES at 1x SBV plus 1/2 SBV group(group E).Hemorrhagic Shock was induced by intermittently withdrawing blood from an iliac catheter.Except group A and B, the other groups were resuscitated with different liquids and lasted 30 min after 1h for shock.The extent of brain edema and inflammatory cells infiltrating was observed by Hematoxylin-Eosin staining,and the expression of nuclear factor kappa-B (NF-κB) in the rat brain and translocation of nucleus were detected using immunohistochemistry and Western blot method. RESULTS: Except the sham group, there were brain edema and inflammatory cells infiltrating in the other groups, which was more obvious in group C and D.The expression of NF-κB in the rat brain was significantly increased in group B and other groups as compared with those in group A (P<0.01);but their increase were markedly decreased in group E as compared with those in C and D groups(P<0.05 orP<0.01).The increase of translocation of NF-κB was nearly accordant with the expression of NF-κB in the rat brain in every group.CONCLUSION: Our results suggest that the expression and the translocation of nucleus of NF-κB in brain tissue were significantly increased in the rat during early hemorrhagic shock (acute blood loss was 35 % of the estimated total blood).6 %HES in the normal doses resuscitation can decrease the expression and the translocation of nucleus of NF-κB, and alleviate the cerebral ischemia-reperfusion injury , and there is the protective effect on the rat brain during hemorrhagic shock.
