丙肝患者与合并HIV 感染者的免疫功能分析

Analysis of the immune function in patients infected with HCV and coinfected with HIV

摘要: 目的: 探讨丙型肝炎合并艾滋病病毒(HIV) 感染者与丙型肝炎病毒(HCV) 单独感染人群的免疫功能差异, 分析其可能的机制。方法: 以28 例丙型肝炎合并HIV 感染者和30 例单纯HCV 感染者为研究对象, 采用第三代ELISA 夹心法检测抗HCV 抗体, 荧光定量PCR 法定量检测HCV RNA, 流式细胞仪计数外周血CD4+、CD8+细胞, 生化分析仪检测肝功能。结果: 28 例合并感染者中有9 例抗HCV 抗体阴性。合并感染者HCV RNA 病毒载量显著高于单纯HCV 感染者, CD4+细胞计数显著低于单纯HCV 感染者, A LT (54.1 U/L ∶ 33.5 U/L, P =0.000)、AST(55.2 U/ L ∶ 35.5 U/L, P =0.018) 均明显高于单独感染组。结论: 检测HCV RNA 及相关免疫功能对于丙型肝炎合并HIV 感染者的治疗、减缓肝病的进展等具有十分重要的意义。

Abstract: AIM: To investigate the immune function difference between HCV infected patients and patients coinfected with HIV and to analyze its potential mechanism. METHODS: 30 patients of HCV infection and 28 patients of coinfected with HIV were studied. The serological anti-HCV antibodies of patients were detected with Enzyme linked immunosorbent assay (ELISA). HCV RNA was detected with Quantitate fluorescence PCR assay. Facs Calibur flow cytometry was used to detect CD4+ and CD8+ cells. RESULTS: There were 9 cases of negative anti-HCV antibody in 28 patients coinfected with HIV. HCV RNA virus load in HIV coinfected patients was more than that in HCV alone infected patients; CD4+ cell was lower than that in HCV infected patients. In HIV coin fected patients ALT and AST levels were raised significantly compared with those in HCV infected patients. CONCLUSION: It is very important to detect HCV RNA and analyze the immune function of HIV coinfected patients for treatment and slowing down advancement of hepatopathy.
