葛根素对兔急性肺栓塞溶栓治疗后单核细胞趋化蛋白-1、肿瘤坏死因子-α含量及肺超微结构的影响

Effect of puerarin on contents of MCP-1, TNF-ɑ and pulmonary ultrastructure after thrombolytic therapy in rabbits with acute pulmonary embolism

摘要: 目的: 探讨葛根素对急性肺血栓栓塞溶栓治疗后单核细胞趋化蛋白-1(MCP-1)、肿瘤坏死因子-α(TNF-ɑ)含量及肺超微结构的影响。方法: 24只日本大耳白兔,随机分为假手术组、单纯溶栓组、葛根素组,经右心导管注入条柱状自体血栓,制备急性肺血栓栓塞实验模型,心导管法监测血流动力学。溶栓后4 h取肺组织,测定肺组织湿干重比(W/D),分别用ELISA及比色法检测肺组织MCP-1、TNF-ɑ含量及髓过氧化物酶(MPO)活性的变化,光镜、电镜下观察肺组织显微超微结构改变。结果: 在溶栓开始后1 h,单纯溶栓组和葛根素组肺动脉平均压(mPAP)即显著下降(P<0.01),且葛根素组mPAP明显低于单纯溶栓组(P<0.05);单纯溶栓组和葛根素组MCP-1、TNF-ɑ及W/D、MPO均明显高于假手术组(P<0.05或P<0.01),葛根素组MCP-1、TNF-ɑ及W/D、MPO均明显低于单纯溶栓组(P<0.05或P<0.01);组织病理及超微结构研究显示,葛根素组血管内皮细胞和肺泡上皮细胞损伤较单纯溶栓组明显减轻。结论: 葛根素可降低急性肺血栓栓塞溶栓后肺组织MCP-1、TNF-ɑ含量,抑制再灌注损伤炎症反应,对溶栓治疗后肺组织有保护作用。

Abstract: AIM: To observe the effect of puerarin on the contents of MCP-1,TNF-ɑ and pulmonary ultrastructure after thrombolytic therapy in acute pulmonary thromboembolism.METHODS: Twenty-four Japanese rabbits were randomly divided into sham operation group, thrombolysis-only group, and puerarin group.Acute pulmonary thromboembolism models of rabbits were established with injection of autologous blood clots through the right heart catheters. Haemodynamic monitoring was performed by introducing heart catheter through right jugular vein. These rabbits were sacrificed at 4 h after thrombolysis, the lung was removed and the ratio of wet to dry of lung tissue weight (W/D) was detected. The contents of MCP-l and TNF-α were also investigated with ELISA. The myeloperoxidase (MPO) activities of lung tissue were detected with colorimetry. The ultrastructure of lung tissue was observed by electronic microscope.RESULTS: The pulmonary arterial mean pressure (mPAP) was decreased immediately at 1 h after thrombolysis both in thrombolysis-only group and puerarin group (P<0.01), the mPAP in puerarin group were significantly lower than those in thrombolysis-only group at 1 h after thrombolysis(P<0.05); The contents of MCP-l and TNF-α, the value of W/D and the MPO activity of lung tissue were significantly increased in thrombolysis-only group and puerarin group than those of sham operation group; The histopathologic analysis showed the injuries of vascular endothelial cells and alveolar cells in thrombolysis-only group were more severe than those in puerarin group.CONCLUSION: Chinese herb puerarin can protect lung against reperfusion injury after thrompolytic therapy in acute pulmonary thrombembolism which may associate with the action of inhibiting inflammation reaction by reducing the contents of MCP-1 and TNF-α.
