Study on the correlation of C-peptide,insulin,cortisol with neonatal hypoglycemia

C肽、胰岛素及皮质醇与新生儿低血糖的相关性研究

Abstract: Objective To analyze the correlation of C-peptide,insulin,cortisol with neonatal hypoglycemia.Methods Totally 258 cases of newborns with hypoglycemia admitted to hospital from January 2015 to February 2016 were selected as the research objects.Another 80 cases of newborns with normal blood glucose were selected as contrast group.C-peptide,insulin and cortisol level were compared between the hypoglycemia group and the normal group to analyze the correlation of C-peptide,insulin,cortisol level,hypoglycemia duration time with hypoglycemia value.Results The levels of C-peptide and insulin of newborns in the hypoglycemia group were higher than those of the normal group,and the cortisol level was significantly lower than that of the normal group (P<0.05).C-peptide,insulin and hypoglycemia duration were positively correlated (r=0.748,0.814,all P<0.001);Cortisol levels were negatively correlated with duration of hypoglycemia (r=-0.809,P<0.001).There was a positive correlation between the values of C- peptide,insulin and hypoglycemia (r=0.832,0.786,all P<0.001) and cortisol level was negatively correlated with the values of hypoglycemia (r=-0.623,P<0.001).Conclusions The occurrence of neonatal hypoglycemia correlate with C-peptide,insulin,and cortisol.For newborns with abnormal levels of C-peptide,insulin and cortisol,special attention should be paid to prevent hypoglycemia though the test of hypoglycemia is normal so as to decrease misdiagnosis and to reduce the incidence of neonatal hypoglycemia sequelae.

摘要: 目的 分析C肽、胰岛素及皮质醇与新生儿低血糖间的相关性。方法 选取2015年1月-2017年1月本院新生儿科收治的新生儿低血糖258例为研究对象,另取正常血糖新生儿80例为对照,对比低血糖组与正常血糖组C肽、胰岛素及皮质醇水平,分析低血糖组C肽、胰岛素及皮质醇水平与低血糖持续时间及低血糖数值间的相关性。结果 低血糖组新生儿的C肽、胰岛素水平高于正常组,皮质醇水平则明显低于正常组,差异均具有统计学意义(P<0.05)。C肽、胰岛素与低血糖持续时间呈正相关(r=0.748,0.814,P均<0.001);皮质醇水平与低血糖持续时间呈负相关(r=-0.809,P<0.001)。C肽、胰岛素与低血糖数值呈正相关(r=0.832,0.786,P均<0.001);皮质醇水平与低血糖数值呈负相关(r=-0.623,P<0.001)。结论 C肽、胰岛素、皮质醇与新生儿低血糖的发生存在相关性;对于C肽、胰岛素与皮质醇含量异常的新生儿,即使血糖检测正常,也应注意低血糖的发生,减少漏诊,及时干预,可以减少新生儿低血糖后遗症的发生。
