AIM: To establish the rat model of ischemic chronic heart failure by coronary artery ligation combining with exhaustive swimming. METHODS: 40 adult rats were treated with coronary artery ligation, after 4 weeks cardiac function measurement were conducted by ultrasonography. Rats with LVEF below 40% are considered as successful model duplication. 11 rats were collected for the coronary artery ligation group, while the rest (whose LVEF were bigger than 40%) were pushed to swim for 1 h per day by 15 days to promote the model formation which 8 rats were collected for exhaustion with ligation group. Left ventricular function indexes, brain natriuretic peptide (BNP) and cardiac histomorphologic changing were checked, and compared with the Control group (10 rats). RESULTS: LVEF of exhaustion with ligation group was (38.70±10.10)%, coronary artery ligation group (39.20±11.10)%, which was obviously decreasing (P<0.01) compared with that of the control group (84.60±3.64)%. LVEDP of exhaustion with ligation group was (11.5±1.3) mm Hg, coronary artery ligation group[(10.68±4.45) mm Hg], which was obviously increasing (P<0.01) compared with that of the Control group[(4.4±0.2) mm Hg]. The BNP level of exhaustion with ligation group was (561.0±21.0) μg/L, coronary artery ligation group (548.6±25.8) μg/L, which was obviously increasing (P<0.01) compared with that of control group[(366.2±21.8) μg/L]. There are lots of red myocardial cells with stripe clear in the control group based on Masson's trichrome staining, but there are so many blue collagenous fibers instead of myocardial cells in exhaustion with ligation group and coronary artery ligation group. The standard-reaching rate of model was about 35% at 4 weeks after operation, while final standard-reaching rate rose to about 62% after exhausting swimming. Although the difference between the indexes of the coronary artery ligation group and the post ligation group was not significant, the rate of improvement was significant (P<0.01). CONCLUSION: Ligation of coronary artery combined with swimming exhaustion can establish ischemic chronic heart failure model, which is more economic and can obtain high success rate, thus is suitable to generalization.

AIM: To explore the effect of M-CSF and its receptor on the response of ovarian stimulation by comparing the expression of macrophage colony stimulating factor (M-CSF) and its receptor mRNA in luteinized granulosa cells in patients after using combinant human follicle stimulating hormone.  METHODS: Ninety-six patients with polycystic ovary syndrome (PCOS) and 157 patients with non-PCOS underwent in vitro fertilization were divided into four groups, i.e. the PCOS fast and slow reaction group, and the non PCOS fast and slow reaction group, according to their response to recombinant human follicle stimulating hormone (r-FSH). Luteinized granulosa cells were then collected after mature follicular puncture. SYBR Green quantitative RT-PCR method was used to detect the expression of M-CSF, M-CSFR and GAPDH in the mRNA gene of the granulosa cells samples. The relative quantity of these genes were determined by comparing the threshold value (CT value of the target gene subtract CT value of housekeeping gene). The difference of gene expression between two groups was compared by t test, and Spearman correlation analysis was used to describe the data relationships. RESULTS: No significant difference was observed in the use of r-FSH among the different groups (P>0.05). Neither was there any significant difference in mRNA quantity of M-CSF or M-CSFR between the entire PCOS and non PCOS patients (P>0.05). After grouping, no significant difference was observed between any two groups in the expression of M-CSF(P>0.05). The expression of M-CSFR in PCOS slow response group was significantly lower than that of PCOS fast response group (P=0.006). Meanwhile, the Spearman analysis showed that the correlation between the quantification of M-CSFR mRNA and the days of r-FSH in PCOS group was statistically significant(P=0.023); the correlation coefficient was 0.511. CONCLUSION: The slow response to ovarian stimulation in PCOS patients is possibly related to the reduction of granulocyte M-CSFR expression. The M-CSF and its receptor may be involved in the ovarian stimulation response process.

AIM: To observe the levels of monocyte-platelet aggregates (MPA) and markers of activated monocytes in patients with unstable angina pectoris (UAP) accepting Ginkgo biloba tablet treatments, and to explore its mechanisms for cardiovascular disease treatments. METHODS: The levels of MPA, CD11b, and MCP-1 were measured in 92 unstable angina pectoris (UAP) and 42 stable angina pectoris (SAP). The UAP patients were randomly assigned into routine treatment group (control group) and combined tablet treatment group (Ginkgo biloba group). The efficacy was assessed, and the levels of MPA, CD11b, and MCP-1 were measured after 28 days of treatment, respectively. RESULTS: The levels of MPA, CD11b, and MCP-1 in UAP group were higher than those in SAP group (P<0.001). The levels of MPA and CD11b were positively correlated with MCP-1 level (P<0.01). The total rate of effective Ginkgo biloba tablet treatment was higher than that of non-Ginkgo biloba tablet treatment (P<0.05). After 28 days of treatments, the levels of MPA, CD11b, and MCP-1 in Ginkgo biloba group were significantly lower than those in control group (P<0.001). In total effective treatment group, the levels of MPA, CD11b, and MCP-1 were significantly lower after treatment than those before treatment(P<0.001), and the decreased rates of these markers after treatment were also much higher (P<0.01). CONCLUSION: There is an obvious efficacy of Ginkgo biloba tablet on unstable angina pectoris by down-regulating the levels of MPA, CD11b and MCP-1.

AIM: To provide rational evidence for the administration of warfarin in clinic by investigating the utilization of warfarin in our hospital.  METHODS: By retrospective analysis, the prescription of warfarin in our hospital was obtained from November 2016 to February 2017 from the system for clinical pharmacist. The general situation of the inpatient, the diagnosis of disease, the use of warfarin, and the combination of drugs and the number of International Normalized Ratio (INR) were statistically and descriptively analyzed. RESULTS:In this investigation, 200 patients were included, of whom 53% were males and 47% were females with an average age of (55.28±13.56) years. Only 21% of the average INR values reached the predicted range (2<INR<3) during hospitalization and the average dose of warfarin was (2.62±0.60) mg. In patients with INR<2, there were 66.92% of patients combined with more than one kind of drugs, which decreased the effect of warfarin. In patients with INR>3, there were 80% of patients combined with more than one kind of drugs, which increased the effect of warfarin. CONCLUSION:The rate of warfarin compliance is low and the warfarin-drug interactions exert a great influence on the patients whose INR value did not reach the predicted range. Thus, to keep the drug safety of patients, more attention should be paid on drug interactions and direct personalized medicine using professional knowledge of pharmacy.

Regulating phase I and II enzyme, drug transporters and constitutive androstane receptor (CAR) play an important role in dug metabolism and keeping balance on the levels of glucose, lipids and hormone in serum. As a hub of material metabolism within body, CAR can affect the efficacy of various drugs and be involved in many common metabolic diseases. Studying its polymorphism is referential for explaining the individual difference genetically and predicting the occurrence and development of diseases. This review focuses on the correlation between the CAR polymorphism and metabolic, which provides evidence for predicting the development of disease and choosing clinical drug dosage.
